What the regulation says

The Romanian framework regulation for schools (ROFUIP, Order no. 5726/2024, art. 157(6)) says the parent of a nursery, kindergarten or primary-school child picks the child up at the end of the programme or authorizes another adult to do it.

The authorized person must be an adult. A minor sibling cannot be authorized.
Each school sets the form of the authorization in its internal rules. Ask the teacher or the office whether they have their own template.
